Guidance Notes for Completing the Application

Individual applications for funding will only be considered by the Trustees if all of the following criteria are met;

1. The beneficiary must have had or is due to have corrective surgery for a cleft lip and/or palate
2. Applicants acting on behalf of the beneficiary (i.e principal carers) must provide clear evidence of their personal means including savings.
3. Clear evidence for the identified support must be provided (outline from a supporting professional)
4. A support plan together with expected outcomes must be provided (outline from supporting professional)
5. Parts 1 and 2 of the application process must be completed in full.

Successful applications

1. Cheques will only be issued, following the receipt of an invoice and will be paid directly to the organisation providing the support or, in the case of travel grants or grants to support residential expenses, to the applicant direct
2. Where appropriate Future Faces will expect a report at agreed points from the supporting professional or their colleague indicating the progress made etc.
3. Future Faces reserves the right to withdraw funding at any stage if any of the information provided is found to be significantly inaccurate.
4. Where the agreed plan is not carried out or prematurely terminated without due consultation to the charity, Future Faces may request the return of some or all contributions made.
